CMS Updates Model Form Requesting Information to Confirm Medicare Status

On April 12, 2021, CMS announced that they have updated the model form requesting information to confirm a claimant’s Medicare status. The previous form asked the claimant to confirm if he/she has ever been enrolled in Medicare Part A or B. The new model form asks if the claimant has ever been enrolled in Medicare and does not specifically ask about enrollment in Medicare Part A or B.
